PCMH Requirements

To become a recognized PCMH you need to have:

• Formal recognition through a nationally recognized organization. NCQA is most well known and accepted.
• Participation in data exchange with AmeriHealth Caritas
• Committed physician leadership
• Committed practice management/administrative staff

The PCMH model of care embraces true integrated care management. It's also a way to improve health outcomes, manage costs, and reconnect with patients for holistic care.
